Mercy Health— Marcum and Wallace Hospital is conducting a comprehensive Community Health Needs Assessment in collaboration with the University of Kentucky's community and economic development initiative.
Community members are encouraged to participate in the assessment by either filling out the online survey or opting for a paper version. This initiative aims to gain insights into the community's health needs, allowing the hospital and its partners to better address those needs.
Taking the assessment only takes 5 to 10 minutes, and all responses will be kept strictly confidential, ensuring your privacy and comfort. To access the survey online, simply scan the provided QR code. For those who prefer a paper version, printed surveys can be mailed to Meghan Mills at Mercy Health—Marcum and Wallace Hospital, 60 Mercy Court, Irvine, KY 40336.
